Patient Fields

Most fields in this section can be completed without assistance or explanation. Some specialized fields, however, are unique, and several built-in features allow for even faster completion of this section and its subsections.

The following tips can help you properly and thoroughly complete each subsection of the Patient section.

Identity

Name
In the first field, enter the first name of the patient. The second field is for the patient's middle name, and the last field is for the patient's last name.

Patient Lookup
After entering the patient's name and/or date of birth, tap the Patient Lookup button to look up the patient within TripTix. If this was a previous patient, the lookup can link to previous run records and auto complete fields within the report, such as those in the Medical History, Address & Contact Info, and Insurance & Payment sections.

SSN
In Windows, tap the Social Security Number Lookup button to gather information about the patient based on their social security number (SSN). In Web, click the search button next to the SSN field to achieve the same results. 

A window opens showing all results for the entered SSN. Select the appropriate individual from the list, and certain fields within the report auto complete, such as those in the Medical History, Address & Contact Info, and Insurance & Payment sections.

Driver's License
In Windows, tap the Barcode Scan button to scan the patient's driver license. Scanning a driver license barcode can auto complete certain patient demographic fields within the report, such as those in the Address & Contact Info section.

In Windows or Web, tap or click the Driver License Lookup button to gather information about the patient based on their driver license number. A driver license number must be entered in the field to the left of this button in order for this feature to function properly.

ID Band/Tag Number
Number on the ID band given to the patient. If an ID band was not used for the patient, select the Not Used check box.

Demographics

DOB or Age
Entering the patient's date of birth in the Date field automatically populates the Age and Units fields if the Actual check box is selected.

If the Actual check box is not selected, you cannot enter the patient's date of birth in the Date field. Instead, the Date field defaults to read-only, showing Not Applicable. To still document the age of a patient, for Age, you can enter the approximate age of the patient and subsequent Units, such as Days or Months.

Weight

Tap or click the Weight Conversion button to have your entry converted into lbs or kg.

Address & Contact Info

Current Location
In Windows, tap this button to auto populate the patient's address fields with your current location.

Note: This button is useful if you are opening the report on scene. The device you are using must have GPS enabled for this feature to work.

Copy from Pickup Address
Tap or click this button to auto populate the patient's address fields with the pickup address.

Note: This button is useful for calls in which the patient was picked up from their place of residence.

Copy to Pickup Address
Tap or click this button to auto populate the pickup address with the patient's address.

Verify Address
Tap or click this button to verify that the address entered for the patient is valid.

Contact Information

Phone
In TripTix Web, enter the type of contact number, followed by the patient's phone number.

Tip: Use the add button to enter additional numbers.

Email
The patient's email address.

Tip: In TripTix Web, use the add button to enter additional emails.

Intermedix Electronic Consent
Whether the patient consents to receiving emails and text messages from Intermedix. Select No for patients who do not wish to receive communications from Intermedix.

Closest Relative or Guardian

Phone
Enter the type of contact number, followed by the guardian's phone number.

Tip: In TripTix Web, use the add button to enter additional numbers.

Name
In the first field, enter the first name of the patient's closest relative or guardian. The second field is for the individual's middle name, and the last field is for the individual's last name.

Copy From Patient
In Windows, tap the Copy From Patient button, directly to the right of the Apt. field, to auto complete the guardian's address fields with that of the patient's address information.

In Web, select the Same As Patient check box directly below the Address field to auto populate the guardian's address fields with that of the patient's address information.

Note: This button is useful for a guardian or relative that lives with the patient.

Intermedix Electronic Contact
Whether the patient's closest relative or guardian consents to receiving emails and text messages from Intermedix. Select No for individuals who do not wish to receive communications from Intermedix.
